1/21/2024 EnchantaVaughn Approved 1.Behavior: Upon arrival to the home setting, Josiah arrived to the session agitated and refused to communicate appropriately with QMHP-C. Josiah had difficulty following the morning routine to put away his things and be seated as directed. Josiah was observed displaying a lot active, impulsive, inattentive behaviors by refusing to engage and getting out his seat without permission and asked on several occasions to focus. Josiah seemed to be distracted by siblings and phone as evidenced by by arguing, fighting and taking each other property. Josiah was hitting and kicking his sibling and struggled with focusing on the discussion at which behaviors last up to 20 minutes in duration. 2. Behavior: Josiah presented playful behaviors throughout the session as he was engaging in other activities with his phone as well as engaging in what his younger sibling was doing at which mother reports behaviors last up to 10 minutes in duration. 1.Intervention: QMHP-C explained how behaviors are counter-productive to achieving personal and treatment goals, and offered Josiah more positive productive options to behaviors. QMHP-C explained the benefits of completing his work, and encouraged Josiah to stay motivated in order to meet specific personal goals. QMHP-C educated Josiah on the A-B-C Model to discuss the connection between behavior and consequences. 1.Response: Josiah continued to display some active, playful behaviors, but was easier to redirect with intervention. Josiah implemented the social skills taught and reinforced in counseling sessions. 2. Response: Josiah had difficulty with self-monitoring and reporting his behavioral progress exhibited through the day. Josiah refused to participate in the discussion regarding behavioral progress and challenges 2. Intervention: QMHP-C provided Josiah to Identify potential triggers to anger and how to intercept behaviors ahead of time. QMHP-C educated Josiah on how to use positive self-talk/affirmations instead of resisting the teacherâs requests as well as his motherâs. QMHP-C educated Josiah on using effective communication to communicate needs appropriately.
